Our core services are NDIS Support Coordination, In-Home Support & Personal Care, Social & Community Access and Home and Living Supports (1:1 SIL & Respite). Every participant is matched with a Care Navigator who gets to know their plan, connects them with the right supports and checks in as things change. Our experienced team also supports people with intellectual and physical disability and behaviour support needs, and our allied health network, with in-house counselling, provides occupational therapy, speech therapy and counselling.

Frequently Asked Questions
What NDIS services does Aus Care Community Services provide?
Aus Care Community Services offers four core services: NDIS Support Coordination, In-Home Support & Personal Care, Social & Community Access, and Home and Living Supports (1:1 SIL & Respite). We also offer Specialist Support: NDIS Hospital to Home Discharge Support, NDIS Community Reintegration Support, and Behaviour Support & Allied Health.
How do I know which NDIS service is right for me?
The right supports depend on your goals, daily needs, and your NDIS plan. Our team can help you understand your plan and connect you with the services that best suit your situation.
Do I need a referral to start services?
No referral is required. You, a family member, carer, support coordinator, or plan manager can contact us directly to discuss services.
Can I use multiple services at the same time?
Yes. Many participants receive a combination of supports such as personal care, community participation, and allied health services, depending on their goals and funding.
How do you make sure support fits me?
Your Care Navigator starts by learning your goals, routines, culture and preferences, then builds your supports around them and reviews them with you as things change, with a focus on dignity and choice.
Do you provide services in participants’ homes?
Yes. Many supports are delivered in participants’ homes, as well as in community settings, depending on the type of service.
What areas do you provide services in?
Aus Care Community Services supports participants across Sydney and NSW-wide.
How do I get started with Aus Care Community Services?
You can contact our team to discuss your needs. We’ll talk through your NDIS plan, your goals, and help set up supports that suit you.
Can families or carers be involved in services?
Yes. With participant consent, families and carers can be involved in planning and supporting services.
Do you support people with complex needs?
Yes. We work with participants with a wide range of support needs, including those requiring behaviour support, specialist coordination, and allied health collaboration.
