      At Aus Care Community Services, we make it easy for NDIS participants, families, support coordinators, and referrers to connect with the supports we provide, including Support Coordination, In-Home Support, Social Support, Respite, Community Reintegration, Behaviour Support, and NDIS Housing Support pathways.

      Where allied health is required (e.g. NDIS occupational therapy, speech therapy, physiotherapy, behaviour support, or NDIS early childhood intervention), we coordinate with external providers to support integrated, participant-focused outcomes.

      Who Can Make a Referral?

      Referrals can be submitted by:

      • Participants (self-referrals welcome)
      • Family members, carers, nominees, and advocates
      • Support Coordinators and Plan Managers
      • Local Area Coordinators (LACs)
      • Hospitals, discharge planners, community services, and other providers
      • Allied health professionals and GPs (when coordination is required)

      We support self-managed, plan-managed, and NDIA-managed participants. If you’re providing a referral on someone’s behalf, we’ll confirm consent and the best contact person during intake.

      What Services Can Be Referred?

      We accept referrals for the NDIS supports we provide, including:

      Coordination and planning

      • Support Coordination (including Support Connection)

      Support at home and in the community

      • Personal Care and In-Home Support
      • Social Support and Group Activities
      • Community Participation and Reintegration Support
      • Respite Care (STA and flexible respite arrangements)

      Specialised supports

      • Behaviour Support
      • NDIS Housing Support pathways (e.g. SIL / SDA / ILO support and coordination)

      If a participant also requires allied health supports (such as OT, speech therapy, or physiotherapy), we can help coordinate with suitable external providers so therapy recommendations integrate well into everyday supports.

      What Information Do I Need to Include?

      To help us assess the referral quickly, it’s helpful to include:

      • Participant details and preferred contact method
      • NDIS number (if applicable) and plan management type
      • What supports are being requested and why
      • Any risks, safety considerations, or support preferences
      • Relevant documents (if available), such as the NDIS plan, reports, or discharge notes

      If you don’t have everything yet, that’s okay, submit what you can and our team will follow up.

      What Happens After I Submit a Referral?

      After you submit a referral, our intake team will:

      • Confirm we’ve received it
      • Review the requested supports and funding alignment
      • Contact the participant (or nominated contact) to confirm details and consent
      • Provide the next steps, timeframes, and an onboarding pathway
      • Coordinate with relevant stakeholders where needed (Support Coordinators, family, external providers)

      We aim to respond within 24–48 hours once the required details are received.
