A Referral Pathway You Can Trust

We welcome referrals from:

• Hospitals and discharge planners
• General practitioners and allied health professionals
• Rehabilitation providers
• Support coordinators and NDIS partners
• Plan managers
• Community organisations
• Participants and families

Referring as a health professional? Read our information for referrers →

• Impairment must be permanent or likely permanent.
• It must significantly limit daily activities.
• A diagnosis alone isn’t enough. Functional impact is what counts.
• Mental health conditions can qualify as a psychosocial disability, but need a permanent and significant impact plus evidence from a treating psychiatrist or psychologist.
• The applicant must be under 65 and meet residency requirements.

If your patient or client already has an NDIS plan, refer them straight to a Care Navigator. No eligibility check is needed. We’ll match them to the right support under their existing funding.

We support two related pathways, so refer to whichever fits:

• NDIS Hospital to Home Discharge Support: for patients being discharged directly from hospital.
• Community Reintegration: for individuals reconnecting with community life after rehab, mental health institutions, corrections, or other long-term institutional care.

• NDIS Support Coordination: help understanding your plan, connecting with providers and making your funding work for you.
  In your NDIS plan: Capacity Building, Support Coordination
• In-Home Support & Personal Care: personal care, meals, household tasks and daily routines in your own home.
  In your NDIS plan: Core Supports, Assistance with Daily Life
• Social & Community Access: support to get out, join activities, build friendships and take part in your community.
  In your NDIS plan: Core Supports, Assistance with Social, Economic and Community Participation
• Home and Living Supports (1:1 SIL & Respite): individualised care, not a shared-support home, plus respite when you need a break.
  In your NDIS plan: Home and Living

• Experienced Care Navigators who know the NDIS and the local service system, supporting participants since 2016
• One Care Navigator, personally assigned to each participant
• Our values of energy, empathy, connection and impact: we return calls and keep referrers in the loop
• We aim to respond within 1 business day and keep everyone updated as the referral progresses
• Experience with complex situations, from hospital discharge and community reintegration to participants previously declined by the NDIS
• Based in Concord, supporting participants across NSW, including Blacktown, Campbelltown, Liverpool, Newcastle, Parramatta and Wollongong
