Whether you’re seeking NDIS occupational therapy, speech therapy, physiotherapy, behaviour support, or NDIS early childhood intervention, we’re here to help.

Our referral process is simple, accessible, and designed to reduce wait times. We understand the urgency that can come with accessing NDIS supports, especially for participants with complex needs. That’s why we prioritise responsive communication, transparent pathways, and a participant-first approach.

What Is NDIS Support Connection?

Support Connection is the first of three levels of NDIS support coordination. It is a short-term service focused on building your ability to understand, navigate, and engage with your NDIS plan and funded supports. While it doesn’t involve long-term case management or crisis response, it equips you with the tools and confidence needed to manage your plan independently.

Eligibility for the National Disability Insurance Scheme (NDIS) services is determined through an assessment process. The NDIS aims to provide support to individuals who require assistance with activities of daily living, social participation, and community engagement due to their disability. It’s important to note that eligibility requirements may vary, so it’s advisable to reach out for personalised guidance.

      Who Can Make a Referral?

      Anyone can refer a participant to Aus Care Community Services, including:

      • NDIS support coordinators
      • NDIS registered providers
      • Local Area Coordinators (LACs)
      • Allied health practitioners and GPs
      • Family members or carers
      • Participants (self-referrals welcome)

      We accept referrals for both plan-managed, self-managed, and NDIA-managed NDIS participants across various support categories.

      What Information Do I Need to Include?

      To ensure your referral is processed promptly, please include the following:

      • Participant’s full name and contact details
      • NDIS number and plan management type
      • A copy of the NDIS plan (or key goals/needs if unavailable)
      • Referrer details (if applicable)
      • Requested services or assessments
      • Relevant documentation or supporting reports (e.g. previous assessments, GP letters)

      You can submit referrals via our secure online form, email, or directly through your organisation’s CRM.

      What Happens After I Submit a Referral?

      Once we receive your referral, our friendly intake team will:

      1. Acknowledge receipt within 1–2 business days
      2. Review the referral and assess eligibility and funding alignment
      3. Contact the participant or their nominee to arrange service commencement
      4. Liaise with support coordinators to ensure the participant’s plan is supported holistically
      5. Book assessments or therapy sessions based on clinical availability and location
